Spinal cord stimulation uses a small implanted device to change how pain signals reach your brain. Fine leads are placed near the spinal cord and connected to a generator roughly the size of a pacemaker. The device delivers mild electrical signals that interfere with pain before it registers.
It is usually considered for people who have already tried the conventional path, injections, medication, physical therapy, and sometimes surgery, without getting the relief they needed.
What spinal cord stimulation treats
The most common reasons we consider it are persistent nerve pain in the legs or arms, pain that continues after spine surgery, complex regional pain syndrome, and painful diabetic neuropathy.
It tends to work best for burning, shooting, electrical nerve pain rather than for mechanical, aching joint pain.
You try it before you commit to it
This is the part that surprises most people, and it is the reason spinal cord stimulation is worth considering even if the idea of an implant makes you uneasy.
Before anything permanent happens, you do a trial. Temporary leads are placed through a needle under image guidance, with no incision, and connected to an external generator you wear for several days. You go home. You do your normal activities. You find out whether it actually helps you.
If the trial gives you substantial relief and you want to proceed, we schedule the permanent implant. If it does not, the leads are removed in a few minutes and nothing has been implanted. You have lost a week and learned something definitive.
Very few treatments in medicine let you test drive them first. This one does.
The permanent implant
The permanent procedure places the leads under image guidance and positions the generator under the skin, usually in the upper buttock or flank. It is done under sedation and you go home the same day.
Recovery involves activity restrictions for several weeks while everything settles into place. After that, you control the device yourself with a handheld remote or a phone app, adjusting the settings for what you are doing that day.
Living with the device
Modern systems are rechargeable or long life, and most are MRI conditional, meaning you can still have scans under specific conditions. Tell any doctor treating you that you have a stimulator.
The device does not cure the underlying problem. It changes how much of the pain reaches you, and for the right patient that difference can be substantial.
